Special Diet Accommodations During Distance Learning
MPS meal boxes do not contain pork, peanuts, tree nuts, or crustacean shellfish. If your child is allergic to milk, egg, soy, or wheat MPS will make every effort to accommodate with alternative options. At this time, MPS is not able to provide full vegetarian boxes, but all boxes will contain some vegetarian items.
To request a special diet box, follow these steps:
Step 1 Complete the online request form for each child in your household that needs dietary accommodations.
Step 2  Ask your child's healthcare provider (Licensed Physician, Physician Assistant, or an Advanced Practice Nurse Practitioner) to complete a Special Diet Statement for each child in your household that needs dietary accommodations. 
  • To ensure your request can be processed in a timely manner, complete the Participant Information section and Voluntary Authorization section (if desired) prior to providing the form to a medical professional who will complete the Required Information: Dietary Accommodation, Additional Information, and Signature sections.
  • Please note that forms completed by Chiropractors, Dietitians, or Health Coaches will not be accepted per MDE’s guidance
  • The form can be returned to MPS by email or by fax (612-668-2830). 
Step 3 After reviewing all documentation, MPS CWS staff will reach out to confirm the type of special diet, your pick up site and start date.
Boxes must be picked up each week! If your special diet box is not picked up for two consecutive weeks, we will cancel the request. If you have a scheduled commitment and know in advance that you will not be picking up your box, please email us so we avoid preparing the box.
